• First Name
• Last Name
• Email Address
• Phone Number
• Address 1
• Address 2
• City
• State/Province/Region
• ZIP/Postal Code
• Country (if outside USA)
• Do you have unopened product you wish to return for credit
• Name of Product Ordered

Follow these steps:

1. Pick up your phone and call (833) 827-5216
2. Tell the rep you need to cancel
3. Give the rep the info they need to find your account
4. Make sure that you won’t be charged again by asking the rep
5. Ask if you can be granted a refund
6. Request that the agent emails you confirmation or gives you a verbal confirmation code
7. Keep the proof given to you for your records
